Fees:
- *$750 for the first initial visit which includes; Family Medicine, Osteopathy, Integrative Medicine , acupuncture
- $450 for follow up visits
- Botox, $400 per site, in addition to the house call fee
- Dermal fillers $600 per full syringe(.8cc) and $400 for half syringe(.4cc), in addition to the house call fee
- Trigger point injections, $125 per injection
Our Policy:
Fee for Service. Receipts will be given at the time of the visit. Cash, bank checks and travelers checks accepted .
*Credit Card payments for established patients only
- .There will be a cancellation fee for any appointment cancellation within 24 hours of your scheduled appointment. This fee will equal the full cost of the appointment.
- There will be a “no-show” fee for any appointment where the Doctor is at your home for the scheduled appointment and you are not .The “no show” fee will equal the full cost of the scheduled appointment.
- We prescribe no more than 1 months supply of a non controlled medication if the medical condition is stable during the first visit.
- Controlled substances are only prescribed if the condition is acute and the period of time is deemed appropriate by the physician.
